At Community Health Alliance, we recognize that mental and emotional well-being are essential to your overall health. That’s why we offer integrated, trauma-informed care that supports the whole person—body, mind, and community.

Our mental health services are led by Transitional Living (TLC), providing comprehensive support including therapy, psychiatric care, and community-based programs. Whether you’re navigating anxiety, trauma, depression, or co-occurring challenges, our providers walk beside you with compassion, expertise, and respect.

What It Can Look Like

  • Individual Counseling

Talk one-on-one with a licensed therapist in a private, judgment-free space. Together, you’ll work on what matters most to you—building coping skills, navigating tough emotions and moving toward the life you want.

  • Group Counseling

You’re not alone. Group sessions bring people together to share experiences, learn from one another, and build a sense of connection. A trained therapist leads each group in a supportive and safe space.

  • Psychiatric Care

If medication or a deeper evaluation is needed, our psychiatric providers are here to help. We work with you to understand what’s going on and support you in finding the right balance.

  • Medication-Assisted Treatment (MAT)

For individuals living with both substance use and mental health concerns, MAT offers a supportive bridge to healing. Though primarily offered through Sojourner, MAT is often coordinated with TLC’s behavioral health and psychiatric services.

  • Primary Care & Nursing Support

Physical health and mental health are deeply connected. Our integrated medical services support everything from routine care to medication monitoring—ensuring you feel well in every way.

  • Residential Treatment (Mental Health Focus)

Sometimes healing needs extra structure. TLC offers 24/7 residential mental health programs that provide a safe place to stabilize, build daily living skills, and work toward independence.

  • Case Management

Case management helps people with mental illness manage their symptoms, stay connected to care, and meet personal goals while living in the community.

Need help keeping everything organized? Our case managers assist with benefits, housing, appointments, and anything else that helps you stay on track.

  • Employment & Education Support

We help individuals overcome barriers to employment and schooling—offering support with training, applications, and confidence building along the way.

  • Forensic & Court-Based Services

TLC partners with local courts, jails, and community agencies to provide mental health support for individuals navigating the justice system, including jail-based services and discharge planning.

  • Recovery Housing

While primarily part of Sojourner’s recovery program, some individuals with co-occurring mental health needs may access supportive sober housing with embedded behavioral health care.

  • Targeted & Specialized Programs

We provide tailored care for pregnant women, people experiencing homelessness, those reentering the community after incarceration, and more. Whether your need is clinical, social, or practical—we meet you where you are.
