At Community Health Alliance, we know that healing doesn’t stop when you leave a counseling session. Real progress happens in everyday life—at the doctor’s office, in the courtroom, on the bus, or at home when you’re trying to make ends meet. That’s where case managers and peer supporters come in.
Our case management services are led by Transitional Living (TLC) and are focused on helping you
navigate real-life challenges with hands-on support. Whether it’s getting to an appointment, checking in with the court, or making sure your basic needs are met, TLC case managers walk beside you in the community to help you stay on track.
At the same time, peer support services are led by Sojourner and offer something just as important:
connection. Peer supporters are individuals in recovery themselves—people who’ve faced similar struggles and now offer encouragement, motivation, and practical wisdom as you move through your own journey.
Together, case management and peer support form a bridge between clinical care and daily life—so you don’t have to figure everything out on your own.

What It Can Look Like
• Transportation & Appointment Support
Need a ride to court, a medical visit, or a pharmacy pickup?
TLC case managers help you get there—literally—so you never have to miss something important because of logistics.
• Medication & Health Follow-Ups
Case managers assist with medication check-ins, refills, and coordination with prescribers—especially for
individuals managing mental health conditions or multiple medications.
• Food, Clothing & Basic Needs
Through access to our food pantry and partner resources, TLC staff help ensure you and your family have what you need to stay healthy and stable.
• Court & Legal Support
Facing probation, child support court, or housing-related legal issues?
We provide coordination and accountability while supporting your dignity and progress.
• Community Navigation & Benefits Support
Case managers help with applications for Medicaid, SNAP, housing vouchers, and ID replacement—taking the guesswork out of systems that can feel overwhelming.

TLC case managers can meet you in the community—not just in an office—making care more flexible,
responsive, and personal.

We won’t let you go without. Case managers connect you to emergency resources and coordinate with CHA’s food pantry and community partners.
Confusion Navigating Social Services
If you’re stuck in paperwork or don’t know where to start, case managers will walk you through benefits applications, court paperwork, and follow-up steps.
Loneliness, Hopelessness, or Isolation in Recovery
When you feel like no one understands what you’re going through, peer supporters step in as a voice of experience and encouragement.
Relapsing or Feeling Discouraged
