Supportive Living

At Community Health Alliance, we know that healing takes more than appointments and prescriptions—it takes stability, safety, and community. That’s why we offer supportive living options designed to help people build or rebuild their lives in environments that encourage recovery, independence, and dignity.

Our supportive housing services are provided through both of our partner agencies. Transitional Living (TLC) leads our mental health residential programs, offering 24/7 care and skill-building for individuals with serious mental illness. Sojourner operates our recovery housing, providing sober, structured living environments for individuals working toward long-term recovery from substance use.

Together, these programs give individuals a place to stabilize, grow, and transition into independent
living—with access to outpatient services, peer support, medical care, and case management built right in.

What It Can Look Like

  • Mental Health Residential Programs (TLC)
    Structured group home and apartment settings for individuals with significant mental health needs.
    Residents receive round-the-clock support, daily living skills training, and access to outpatient therapy and medical care.

  • Recovery Housing (Sojourner)
    Safe, sober living environments for individuals in recovery from substance use.
    Residents follow a structured routine, engage in outpatient treatment, and receive support from peer mentors and on-site staff.

  • Family-Focused Housing for Mothers & Children
    Specialized recovery housing that allows pregnant or parenting women to receive treatment while living with their children. Services include parenting support, childcare, and access to programs like MAMAS.

  • On-Site and In-Home Support
    Many homes include 24/7 staff presence, helping with crisis response, medication reminders, or just someone to talk to. This stability helps residents focus on building healthy routines.

  • Access to Counseling, Medical, and Peer Services
    Residents are connected to the full range of CHA services—including therapy, primary care, MAT, and peer
    support—without needing to leave their living environment.

  • Transitional Planning

    When you’re ready to move on, we’ll help you build a plan for the next step—whether it’s independent housing, supportive services, or reuniting with family.
