We believe everyone deserves access to high-quality care. At Community Health Alliance, we accept a wide range of insurance plans and offer multiple options to ensure cost is never a barrier to your wellbeing. Whether you’re navigating Medicaid, Medicare, or private insurance, we’re here to help.
If your plan isn’t listed below or you’re not sure whether you’re covered, just give us a call. Our billing team is always happy to confirm your benefits and help guide you through next steps. For some situations, we may even be able to establish a single case agreement or check out-of-network benefits.
In-Network Insurance Providers
ACA Marketplace Plans
We are in-network for most ACA Marketplace (Obamacare) plans, including:
- Aetna
- Centene (Buckeye
- Health Plan)
- AmeriHealth
- Caritas
- Anthem Blue
- Cross Blue Shield
- Ambetter
- CareSource
- Humana
- Molina
- Healthcare
- UnitedHealthcare
- Cigna
All plans are listed as ACA Marketplace Plans. For questions about specific plan types, call our billing team.
Medicaid & MyCare Ohio Plans
We accept many Medicaid and MyCare plans across Ohio. Participating plans include:
- Aetna
- AmeriHealth
- Caritas
- Anthem
- Buckeye Health
- Plan
- CareSource
- Medical Mutual
- (SJNR only)
- Humana
- Molina
- UnitedHealthcare
- Ohio Department of Medicaid
Please note: MyCare plans are being phased out by 2026 and may shift to Medicaid-only coverage.
Medicare and Advantage Plans
We are in-network for select Medicare Advantage and MyCare plans such as Aetna MyCare, though some Advantage plans are not accepted or are out-of-network only. We recommend calling our billing department before scheduling if you have:
- A DSNP (Dual Special Needs) plan
- A Medicare Advantage plan not listed on our accepted list
Local and County-Specific Programs
We also work with county mental health boards and grant-based payers, including:
- Butler County Mental Health & Addiction Recovery Services Board
- Preble, Warren, Clinton Counties Mental Health Recovery Boards
- Department of Mental Health & Addiction Services (Ohio)
- Opportunities for Ohioans with Disabilities (OOD)
Services may be restricted to specific programs like Room & Board, Drug Court support, or outreach funding. Authorization and pre-approval may be required.
