Community Health Alliance offers integrated care across 15 locations in Butler, Clinton, Warren, and Preble counties, with Telehealth available statewide.

Several locations provide direct community access with walk-in services for mental health, primary care, recovery, and more. Other sites support clients through referral-based care and coordinated services, including housing and specialized programs.
