Integrated Primary Care
At Community Health Alliance, we believe physical health is a key part of emotional and behavioral wellness. That’s why we offer primary care as an integrated service within our mental health and recovery programs—so you can get the support you need, all in one place.
Our medical services are provided by both Transitional Living (TLC) and Sojourner, depending on where
you receive your care. TLC integrates primary care and nursing support into mental health treatment, while
Sojourner provides medical services as part of recovery programs, including medication-assisted treatment
(MAT) and residential care.
Whether you’re managing chronic conditions, responding to new symptoms, or monitoring medications, our
medical team works alongside your therapist, case manager, or peer supporter to keep your body and mind in sync—because healing happens best when we treat the whole person.

What It Can Look Like
• Routine Checkups & Preventive Care
Keep track of your physical health with regular medical visits.
Our clinics offer blood pressure checks, labs, screenings, and general wellness monitoring—all while staying
connected to your treatment plan.
• Chronic Condition Management
Get support for diabetes, high blood pressure, asthma, chronic pain, or other long-term health conditions.
We’ll work with you to manage symptoms, reduce stress, and avoid complications that might disrupt your
progress.
• Medication Management
Medical staff help monitor prescriptions for mental health, MAT, or chronic health issues—ensuring that your medications work well together and support your overall wellness.
• MAT Support (Recovery-Focused)
For individuals in recovery, our providers offer safe, evidence-based care using FDA-approved medications like methadone, buprenorphine (Suboxone), and naltrexone (Vivitrol)—paired with counseling and care
coordination.
• Health Monitoring in Residential Programs
If you’re staying in one of our mental health or recovery residential programs, nursing staff are available to help with daily check-ins, vitals, medication support, and urgent concerns.
• Education & Health Coaching
Our team can help you better understand your diagnoses, treatment options, and how to take care of your
body—especially if health hasn’t always been a priority.

connect you to a provider on-site—so you don’t have to guess or go somewhere unfamiliar.

When physical conditions like diabetes, pain, or high blood pressure start affecting your treatment or mood, we’ll help manage both—right here, with your full care team involved.
Side Effects or Medication Concerns
Whether you’re taking psychiatric medication, MAT, or something for a physical condition, we monitor for side
effects and coordinate adjustments with your provider.
Pregnancy-Related Health Needs
If you’re pregnant or parenting and share a concern, we can quickly connect you with medical care, perinatal
education, or programs like MAMAS—all while staying aligned with your overall treatment plan.
Stress-Related Health Symptoms
If emotional stress is causing headaches, GI issues, sleep problems, or high blood pressure, our integrated team can help address the root causes and physical effects together.
